Service Coordination
Our Support Coordinators have the skills and knowledge to get resources to appropriate services. They are friendly and approachable and guiders to create a person center planning tool. They are advocates that will act as a point person for all services. They are responsive and available to discuss any service-related concerns. They are efficient, well organized, and competent.
